The steps included streamlining oxygen distribution,\u00a0active\u00a0case\u00a0finding\u00a0in\u00a0rural\u00a0areas, and health\u00a0systems\u00a0strengthening.<\/p>\n<p><strong>Oxygen Distribution<\/strong><\/p>\n<p>Oxygen,\u00a0being one of the most critical elements for clinical treatment, had to be managed at\u00a0a war footing. For this, the focus was on the most important metric \u2013 location of assets\u00a0and managing\u00a0the movement of assets to carry as much oxygen as possible, with an underlying focus on optimisation.<\/p>\n<p>The State Government activated the teams all over UP and other sourcing states\u00a0like Jharkhand, West Bengal &amp; Odisha to intercept the trucks carrying oxygen cylinders,\u00a0deployed an Application that was downloaded on smart phones- which were then placed in\u00a0each\u00a0of\u00a0the\u00a0trucks\u00a0plying,\u00a0without\u00a0disturbing\u00a0their\u00a0ongoing\u00a0route.\u00a0Simple\u00a0SOP was\u00a0to\u00a0be\u00a0followed- to track their live\u00a0location.\u00a0This was\u00a0called the\u00a0stealth approach\u00a0\u2013 speed and scale beats optimal and perfect anytime, in a war-like\u00a0situation. Key macro-decisions undertaken were to deploy the rail network to bring oxygen\u00a0supplies from the eastern states to UP and airlift the empty cylinders with the help of the Air\u00a0Force, using\u00a0airports like\u00a0Agra, Hindon\u00a0&amp; Lucknow\u00a0back\u00a0for re-filling.<\/p>\n<p>The information from App was fed into a live dashboard, which was able to display all the trucks\u00a0carrying Oxygen &amp; helped the decision makers to address the allocation gap smartly and\u00a0swiftly and stabilise the oxygen supply.\u00a0Real time information on tankers and\u00a0their movement, being the most critical element of the O2 supply chain, was displayed using\u00a0the dashboard to decision makers. The dashboard called \u201cOxyTracker\u201d was used to visually\u00a0flag\u00a0the\u00a0efficiency\u00a0of\u00a0the\u00a0entire\u00a0fleet\u00a0with\u00a0one\u00a0sweep\u00a0of\u00a0the\u00a0eye.\u00a0Key\u00a0elements\u00a0displayed\u00a0about\u00a0tankers<\/p>\n<p>include:\u00a0\u201crunning\u201d,\u00a0\u201cidle\u201d,\u00a0\u201ctransit\u00a0empty\u00a0or\u00a0full\u201d,\u00a0\u201cETA\u00a0to\u00a0destination\u201d,\u00a0\u201cdriver\u00a0name\u201d\u00a0and\u00a0\u201cnumber\u201d and\u00a0\u201cquantity\u201d being\u00a0carried,\u00a0\u201ctankers\u00a0travelling\u00a0by\u00a0plane,\u00a0train\u00a0or\u00a0road\u201d.<\/p>\n<p>5 major hubs were created in UP \u2013 Modinagar, Agra, Kanpur, Lucknow and Varanasi as the\u00a0primary and then Bareilly and Gorakhpur as the secondary hubs for optimising the\u00a0entire supply chain. These hubs\u00a0fed the regions around them, within the cycle time limit of 10 hr max.\u00a0It ensured that all the tankers arriving within these hubs have to be shipped within\u00a0these regional centres and distribute the oxygen at designated places and reach the\u00a0airports\u00a0\u2013\u00a0within\u00a010\u00a0hrs. Each of these hubs have the airports &amp; these airports were used to bring the tankers\u00a0back\u00a0to\u00a0shipping\u00a0points\u00a0\u2013places\u00a0where\u00a0UP\u00a0has\u00a0been\u00a0allocated\u00a0the\u00a0gas\u00a0quota\u00a0\u2013\u00a0Jamnagar,\u00a0Jamshedpur,\u00a0Bokaro,\u00a0Durgapur,\u00a0Haldia\u00a0and\u00a0few\u00a0other\u00a0centres\u00a0in\u00a0West\u00a0Bengal.\u00a0All\u00a0the shipping centres are within the earshot of the airports too. The airlifts saved 40%\u00a0in\u00a0cycle\u00a0team.<\/p>\n<p>Once the empty tankers are back in batch of two, get them filled as soon as possible, put them quickly on rail\u00a0yard\u00a0for\u00a0loading\u00a0and\u00a0hopping,\u00a0in\u00a0a\u00a0batch\u00a0of\u00a04.\u00a0Ship\u00a0the\u00a0train\u00a0with\u00a0rakes\u00a0with\u00a04 tankers\u00a0on it.\u00a0Each\u00a0train\u00a0to carry\u00a0at\u00a0least\u00a080\u00a0MT\u00a0to\u00a0dedicated\u00a0hub\u00a0point. Once the train leaves, based on from where, one will start receiving trains at one of\u00a0the\u00a0hubs\u00a0within\u00a016-22\u00a0hrs range,\u00a0based\u00a0on\u00a0shipping\u00a0point\u00a0and the\u00a0hub. Once\u00a0the\u00a0hub\u00a0receives\u00a0the\u00a0tankers,\u00a0run\u00a0them\u00a0on\u00a0set\u00a0routes\u00a0within\u00a0the\u00a0zone\u00a0to\u00a0keep\u00a0the\u00a0cycle time within 10 hrs to provide ample time for the Airforce to manage the queue.\u00a0Airforce needed to operate 16 flights per day between hubs and shipping centre\u2019s\u00a0details.<\/p>\n<p>The approach helped the UP\u00a0Government\u00a0to\u00a0lift\u00a0almost\u00a01000MT\u00a0in\u00a0about\u00a010\u00a0days\u00a0from\u00a0the\u00a0time.\u00a0Earlier,\u00a0UP\u00a0was\u00a0lifting\u00a0and\u00a0distributing<\/p>\n<p>250MT. Other factors\u00a0that\u00a0contributed\u00a0heavily towards\u00a0the\u00a0outcomes\u00a0included Responsive\u00a0leadership; Speed\u00a0of\u00a0decision\u00a0making; Cross\u00a0functional\u00a0teams\u00a0collaborating\u00a0with\u00a0nimbleness\u00a0and agility; SOPs\u00a0being\u00a0developed\u00a0and\u00a0deployed\u00a0very\u00a0quickly\u00a0in\u00a0the\u00a0field; Operationalizing\u00a0\u2013\u00a0people,\u00a0process\u00a0and\u00a0technology\u00a0triad; and Decoding\u00a0\u201cMassive\u00a0Supply\u00a0Chain\u201d\u00a0complexity\u00a0metrics.<\/p>\n<p>In order to build\u00a0Oxygen Generation\u00a0Capacity, Government\u00a0of India has sanctioned 14 PSA plants for 14 institutions, of which 5 plants have already been\u00a0installed and remaining 9 are under implementation. State Government has issued Purchase\u00a0Orders for 4 PSA plants. These are in addition to the existing working oxygen generation plants in 13 districts. Further, the State Government is constantly engaging with the Government\u00a0of India for identification of sites for establishment of additional PSA plants under PM CARE,\u00a0and a list of 167 additional sites has been shared with Government of India. UP used\u00a0this\u00a0exercise\u00a0to\u00a0develop\u00a0a\u00a0blueprint\u00a0for\u00a0public\u00a0health\u00a0emergency\u00a0plans\u00a0that\u00a0can\u00a0be\u00a0used\u00a0for effective, efficient &amp; timely delivery of oxygen &amp; life-saving drugs or supplies, in the face\u00a0of\u00a0future\u00a0COVID-19\u00a0waves or\u00a0a\u00a0public\u00a0health\u00a0crisis.<\/p>\n<p><strong>ACTIVE\u00a0CASE\u00a0FINDING\u00a0IN\u00a0RURAL\u00a0AREAS<\/strong><\/p>\n<p>The World Health Organization has lauded the efforts of the Uttar Pradesh government, for\u00a0\u201cgoing\u00a0to\u00a0the last mile\u201d\u00a0during\u00a0the\u00a0current\u00a0Covid-19\u00a0crisis.\u00a0 It tweeted\u00a0\u201cThe Uttar Pradesh state government has initiated house-to-house active case finding of\u00a0COVID-19 in rural areas to contain transmission by testing people with symptoms for rapid\u00a0isolation,\u00a0disease\u00a0management\u00a0and\u00a0contact\u00a0tracing.\u201d<\/p>\n<p>In Uttar Pradesh, Government teams have been moving across thousands of villages over several days.\u00a0Each monitoring team has two members, who visit homes in villages and remote hamlets to\u00a0test everyone with symptoms of COVID-19 using Rapid Antigen Tests (RAT) kits. Those who\u00a0test positive are quickly isolated and given\u00a0a\u00a0medicine\u00a0kit,\u00a0with\u00a0advice\u00a0on\u00a0disease\u00a0management.\u00a0All the contacts of those who test positive are quarantined and tested using an RT-PCR at\u00a0home\u00a0by\u00a0a\u00a0rapid\u00a0response\u00a0team.\u00a0Two\u00a0mobile\u00a0vans\u00a0have\u00a0been\u00a0allocated\u00a0to\u00a0each\u00a0Block\u00a0within\u00a0a\u00a0District in the state to test people with symptoms, even as routine sample collection and\u00a0testing\u00a0continues in\u00a0Community Health\u00a0Centres. The state government has deployed 141,610 teams and 21,242 supervisors from the state\u00a0health\u00a0department\u00a0for\u00a0this activity\u00a0to\u00a0ensure\u00a0all\u00a0rural\u00a0areas\u00a0are\u00a0covered.<\/p>\n<p>Those with symptoms are tested and given medicine kits and information on quarantining\u00a0and isolation, both at home and in hospitals. People without signs of COVID-19 are urged to\u00a0get vaccinated and follow COVID-appropriate behaviours to prevent transmission in rural\u00a0areas of India\u2019s most populated state. Micro planning, house visits, concurrent monitoring\u00a0and\u00a0follow\u00a0ups\u00a0were\u00a0core\u00a0to\u00a0India\u2019s\u00a0polio\u00a0eradication\u00a0strategy\u00a0to\u00a0ensure\u00a0no\u00a0one\u00a0misses\u00a0access\u00a0to vaccination\u00a0and\u00a0health\u00a0care\u00a0services.<\/p>\n<p><strong>HEALTH\u00a0SYSTEMS\u00a0STRENGTHENING<\/strong><\/p>\n<p>At present there are around 65 thousand Isolation beds available in various Covid hospitals in\u00a0the\u00a0state.\u00a0There are over 15 thousand beds with ventilators \/HFNC\/ BiPAP, available in various Covid hospitals in state. Around 80 thousand beds have been made\u00a0available\u00a0for\u00a0the\u00a0treatment\u00a0of\u00a0Covid-19\u00a0patients\u00a0in\u00a0all\u00a0L1,L2\u00a0and\u00a0L3\u00a0hospitals.\u00a0There\u00a0are\u00a0total 209 level 2 and\u00a0level\u00a03 hospitals\u00a0available for treating Covid patients in the\u00a0state.\u00a0307\u00a0private\u00a0hospitals are also\u00a0treating\u00a0Covid patients in\u00a0the\u00a0state. In every district, 2 CHCs have been converted into 50 bedded Covid L1 plus hospitals.\u00a07,500\u00a0Beds\u00a0have\u00a0been\u00a0added\u00a0in\u00a0rural\u00a0areas\u00a0by\u00a0opening\u00a0these\u00a0Covid\u00a0treatment\u00a0centres.\u00a0A\u00a0500-bed Covid\u00a0Hospital\u00a0has\u00a0been\u00a0opened\u00a0at\u00a0Lucknow\u00a0with\u00a0the\u00a0support\u00a0of\u00a0DRDO.\u00a0Another\u00a0700 bedded hospital\u00a0has\u00a0been\u00a0started in\u00a0Varanasi\u00a0through\u00a0DRDO.<\/p>\n<p>Almost 1.5 Crore persons in the state have been administered Covid vaccine, three-fourth of which have got one dose while over 30 lakh have taken\u00a0both\u00a0doses\u00a0of\u00a0the vaccine. The State has decided to\u00a0vaccinate the beneficiaries free of cost. CVCs have been identified and State has\u00a0started\u00a0procuring\u00a0vaccines\u00a0for\u00a0the\u00a0above-said\u00a0group\u00a0as\u00a0per\u00a0GoI\u00a0guidelines.\u00a0In\u00a0addition,\u00a0the private CVCs who have procured vaccines from other than the GoI channel, have\u00a0also been registered on the Cowin portal as per guidelines and\u00a0are conducting vaccination.
